REGISTRATION OF CERTIFICATE OF LIVE BIRTH
(COLB-LEGITIMATE) – TIMELY
The Local Civil Registry Office is tasked to accept, file, keep and preserve registrable documents affecting the civil status of a person pursuant to Republic Act No. 3753, otherwise known as the Civil Registry Law.
A child has the right to a name and identification and should be registered within 30 days after birth as evidenced by the Certificate of Live Birth (COLB). Beyond this period, the Local Civil Registrar will issue a delayed registration of live birth to ensure that the child’s right is protected.
If requirements are complete, this transaction can normally be completed in eleven (11) succeeding days. On-time registration could even be shorter.
Who May Avail of Service:
- Informant-client (mother/father/guardian) or their authorized representative.
- Attendant-at-birth or hospital liaison officer if the child is born in a maternity clinic/hospital.

- Parent(s) of child.
- Person himself/herself and at least 18 years old.
- Authorized Representative.
REQUIREMENTS FOR THE DELAYED REGISTRATION OF BIRTHS
1. Four (4) copies of the Certificate of Live Births duly accomplished and signed by the attendant at birth (hilot, midwife, nurses, doctors etc…)
2. Accomplished Affidavit for delayed registration at the back of the Certificate of Live Birth by the father, mother, guardian and registrant to be subscribed by an attorney.
3. Any two of the following documentary evidences which may show the name of the child, date and place of birth and the name of mother and father.
a. Baptismal Certificate
b. School records (nursery, kindergarten or preparatory) (Form 137)
c. Income Tax Return of parents or registrant
d. Insurance Policy
e. Medical Records
f. Voters Registration (COMELEC)
4. Affidavit of two (2) disinterested persons who might have witnessed or known
the birth of the child.
5. Marriage Contract / Marriage Certificate of parents of the registrant
6. Community Tax Certificate (cedula) of the registrant
7. Negative Result from PSA San Fernando Pampanga
